Our Policies

Clinic policies are in place to ensure children receive the care they need in a timely and acuity-based manner. By choosing our clinic, you agree to the policies below.

Confidentiality

Your child’s health information is private and confidential. Unless required by law, any details regarding your child’s health and treatment will not be released without your consent.  

 

Please do not send any personal health information in email form; email contact is strictly for booking purposes or sending forms with no patient identifiers.

Communication with administrative staff is against provincial privacy regulations; please communicate all medical concerns with the physician or nurse at your appointment.

Late/No Show/Cancellation Policy/Clinic FEES

Our clinic is very busy and these policies are in place in consideration of children waiting to be seen. We ask that all our patients are respectful of the following policies:

*All fees are CASH ONLY*

 

  • No Shows: If you do not arrive for your scheduled initial consultation, you will be charged a “no show fee” of $80. Your appointment will not be rescheduled until this fee is paid.

  • Rescheduling/Cancelling: We require 48 hour cancellation or rescheduling notice for all follow ups and 72 hours for all initial consultations to avoid fees. A $40 fee will be charged if notice is not given within the above parameters.

  • If you are more than 20 minutes late for your appointment, it will be rescheduled.

  • Letter fees:

    • Disability Tax Credit - $50

    • School, diagnostic and support letters - $35

    • Daycare notes - $25

Preparing for Your Appointment:  

  • You may be sent a form or forms using our “Ocean” platform. Please complete these forms electronically at least 1 day prior to your scheduled appointment.

  • To access the Ocean form, click the link in your email that says “Complete Questionnaire”. Under verify your identify, please enter your child’s date of birth. Click next. The form will appear. Please click finish on the bottom right of your screen to submit the form. Please complete ALL questions in the form. If a question does not apply, please enter N/A.

  • Please arrive 10 minutes prior to your scheduled appointment; there may be forms that need to be completed in person.

OHIP Covered Services

If you have a newborn, please register with OHIP as soon as possible to obtain a permanent OHIP card (with a version code). The OHIP paperwork provided initially is only temporary and does not allow for your child’s services to be billed.

 

You are responsible for ensuring your child’s health card is up to date and valid. Please check your child’s health card prior to visiting with us and bring their card to each scheduled appointment.

Communication

We are here to help you and your children and we understand having a sick child can be stressful. However, we do not tolerate inappropriate language or intimidating behaviour towards our staff.  Please be respectful when communicating with front staff.

Pediatric & Breastfeeding Clinic Booking Policy

Once a referral is received for your child, our team will triage the referral based on urgency. You will be contacted with a scheduled appointment for an initial consultation with the pediatrician.
